Introduction

My reviews of the community uncover health issues that are undermining overall public health. Despite efforts to identify and address health performance indicators, certain aspects appear to be neglected. Health issues that need to be addressed include the opioid crisis, drug and alcohol abuse, mental health, diabetes, cardiovascular disease and obesity, especially among minors. In general, appropriate interventions must be designed and implemented for the identified health problem. For example, more attention and resources are needed to address cardiovascular disease and diabetes in the community. In this paper, the data collected during the community assessment are reviewed and discussed. It will identify strengths and weaknesses of community care, identify areas of health concern, and select appropriate health targets from the Office of Disease Prevention and Health Promotion (ODPHP, 2014). Heart disease is listed as the leading cause of death in the United States. The term refers to diseases that affect the cardiovascular system. Different types of heart disease affect blood vessels and the heart differently. The most common form of heart disease is coronary artery disease (CHD), which causes the heart muscle to receive insufficient oxygen-rich blood.
